    CHILDREN’S LITURGY
     Family Membership Application and Annual Consent Form 2017 – 2018

    Declaration

    • I agree to the above mentioned child/children taking part in the Children’s Liturgy during Mass at St Joseph’s Catholic Church, Bracknell and I agree that I will accompany any of my children aged 3-5 to Children’s Liturgy.
    • I understand that the children will be encouraged to join in and stay for the duration of the session.  However, if my child wishes to leave before the end, they will be accompanied back into the church to re-join me.  
    • I understand that the Children’s Liturgy sessions are aimed at those children who have not yet made their First Holy  Communion.  I understand that my child will be expected to maintain a good standard of behaviour, appropriate to their age group.  If they are unable to do so, I accept that I may be asked to collect them from the Children’s Liturgy session.   
    • To help establish the numbers attending at each Children’s Liturgy session and to meet safeguarding and fire safety requirements, I agree to obtaining an attendance ticket for each person attending Children’s Liturgy, prior to the start of Mass, which can only be collected upon presenting a valid membership card in the name of each person attending.  I understand that the attendance tickets are limited in number per session and given out on a first come first serve basis.  They cannot be reserved and are non transferable. 
    • Membership is not a guarantee of attendance at Children’s Liturgy.  Registration for Children’s Liturgy will close five minutes before the beginning of Mass. Only those who have been issued with an attendance ticket before registration closes will be allowed entry into that week’s Children’s Liturgy.    
    • I understand that in the event of my child experiencing illness or accident, every effort will be made to contact me.  If for any reason this is not possible, I agree to my child receiving medication as instructed and any emergency medical    treatment as considered necessary by the medical authorities present.
    • I understand that attendance data will be kept by the Diocese in accordance with Child Safety guidelines and will not be used for any other purpose, such as School Admissions or for the First Holy Communion programme.
